[abc] ecoop paper

I have played with the paper a bit more - but am going to give
it a rest until early tomorrow morning.

Here is what I did - everyone should check that it is ok/correct.

I added a bit more to the intro. Someone needs to go through and
tighten my prose (not my strongest point).

I added some more stuff about challenges to the ITD section and
uncommented some material we had commented out to save space. There
is actually some more stuff there that could be uncommented. Oege I
think you should look at this section and see if I messed it up.

I uncommented some things about synthetic advice in the advice section
(these were actually in the \fullreport, but not in \shortpaper). We
still need to add something about pointcuts with alternative bindings
in this section (Ganesh?).

I reworked the beginning of related work so as not to repeat what is
in the new section 6.

I still need to rework conclusions, but I would sort of like to see
the whole paper before doing that.
