[abc] paper updated

I think that we have addressed most of the points I mentioned in
my previous e-mails. I don't have the Laddad textbook, to I can't
do the following one:

Connection management example

- give page numbers in citation to Laddad's textbook?

Ondrej and I had a few e-mail discussions and he has reworked the optim.
section based on that.

I added a few small fixes here and there. Aske fixed the primitive type
stuff.

Pavel, are there any updated performance numbers? I remember that you
were working on a further improvement, did that work out. Also,
Jennifer sent e-mail a bit back saying that the newer version of Soot
fixes some j2j inefficiencies.

When we think we are not adding any more, we should make a complete
read through.

Who is submitting the final version? Will Oege be back by then?
