Hi Lisa,

The problem was with AOSD 2004, but we noticed it when authors
were missing on a preliminary program for the conference and
got it fixed before anything was printed, so all was fine in
the end. It's possible that
it was only a problem with the material produced by the
conference organizers, and not with the proceedings, since I
guess you use a different system to track the authors.

For the conference organizers, they use info collected
by the conference systems like CyberChair, which often have
a max of 6 authors.
