update on patch, Java, C# and C++ XSLT generation

I've done some changes systemizing and packaging
the software.

The changes include:
    - integreated all xsl-s into one package
    - c++ generation is now in xsl instead of java
    - added a script for convenient output generation
    - added loads of README-s everywhere
    - some small bug fixes in stylesheets

All instructions and packages are now up on my sablecc web page
