[abc] POPL paper changes

This email is addressed to all authors of the POPL paper, but primarily to
Pavel, Julian and Neil.

The deadline is this coming Thursday, so we should act now and quickly.

After a discussion this Oege, he said that he would like to think a bit more
carefully what kind of example we may need to add as a good starting point
of the more complex machinery. So we can leave that aspect for the moment
and concentrate on eliminating all the minor corrections. We should not care
for the moment of going above the 12 pages limit.

I suggest that each of us takes a reviewer and deals with all the minor
corrections of that reviewer. Preferably, we should write how we are going
to deal with each comment in an email (for everyone to see and agree),
before actually applying the proposed changes. I am going to start with the
reviewer #1.

Hope we shall finish by tomorrow and can then start addressing the major
issue(s).
